SCHOLARSHIP APPLICATION

Thanks to an anonymous donor, one scholarship will be awarded this year to a player who embodies qualities that CMH Soccer values.

Fill out the application and submit before 05/01/2024 to be considered.

The Scholarship will be awarded May 15th and will cover cost of CMH travel fees for one program and the cost of the basic uniform kit.

Tryout fee not included.

INJURY REPORT FORM

Before completing this form, all injuries should be reported to a member of CMH Soccer's staff.

Fill out the form in it's entirety so a report can be filed on behalf of the player for secondary medical coverage.

HEALTH & SAFETY

Soccer players often shake hands, share water bottles, and are in close proximity of each other before and after practice and games

PLAYER FORMS

All players must complete the following waivers and releases after registration to be eligible for participation with CMH Soccer.

MEDICAL RELEASE

The MEDICAL RELEASE is a required form for participation in any US Club Soccer game or tournament. Guidelines require proof of this form prior to the start of any game or tournament.

Failure to complete this form will result in a player being deemed ineligible to play.
